- I give my permission for my child to attend the Supervision Centre from 1.40pm-2.50pm on the days selected above.
- I understand that my child will be supervised only and not provided any tutorials or learning opportunities during this time.
- I confirm that if my child is booked to attend the Supervision Centre but will not attend on a particular day, I will contact the school administration staff to advise of this.
- I understand that the college will not be responsible for following up absent students from the Supervision Centre. Families will be advised by a text message from the Attendance Officer if their student does not attend the Supervision Centre.
